Meningitis Booster Vaccination Information

If you need to have a meningitis booster shot, you would have received a letter to your home and an email from the Wellness Center. Please disregard this notice if you did not receive this information.

New residential students:
If you are a new student to Stevenson University or if you haven’t been a Stevenson University student for at least 2 years and you are reapplying for admission to Stevenson University as a full-time residential student, you will need to complete another Student Health Form by August 1st for fall entrance or January 2nd for spring entrance. If you have been a student at Stevenson University and you are currently/planning on living on-campus, please follow instructions for “Returning residential students.

Returning residential students:
*If you did not receive an email/mailed letter regarding the meningitis booster vaccination, please disregard this notice.*

Per Section 18-102 of the Health General Article of the Annotated Code of Maryland and Code of Maryland Regulations (COMAR) 10.06.05: Meningococcal Vaccination Requirements for Students in Institutions of Higher Education, you are required to have the meningitis vaccine within 5 years of each move in on campus/during your entire residency on campus. Since your primary dose of meningitis was administered before your 16th birthday, a booster is required before moving into residence on-campus. Housing students will not be permitted to move into residence unless the Wellness Center has proper documentation (ex. updated immunization record or physician script with students name and date of vaccine) by August 1, 2011 for Fall semester and January 2nd for Spring semester.

The Wellness Center is administering the meningitis vaccine beginning April 4, 2011. Call the Wellness Center at 443-352-4200 to schedule an appointment. Insurance will not be accepted but a receipt can be printed for you to submit it to your insurance company. The cost of the shot is $110.00. Contact your primary care provider or various alternative immunization locations if this is not convenient.

Complete this Additional Health Record Submission Sheet and attach your meningitis booster shot documentation. Documentation can be mailed, dropped off or faxed to the Wellness Center. The Wellness Center is located on the Owings Mills campus in the Caves Sports and Wellness building. Our address is 100 Campus Circle, Owings Mills, MD 21117. Please note that we cannot guarantee the arrival of mailed documentation. We highly suggest students make a copy of their documentation for their own records. You may also fax us your form to 443-352-4201. You do not need to mail/drop off the original documentation if you fax a copy.
